Description
I'd like to implement on online registration page for a youth sports organization. Customer payment will be handled by Paypal using IPN. Please only bid AFTER you have fully read the project details below.
For the customer (registrant):

Go to online registration page.
Customer can register one or multiple players. Data needed:

create login id/password
parent/guardian first and last names
street Address
email
telephone
will they like to sponsor - (details to be disclosed)
would they like to coach a team(s) (details to be disclosed)
read and accept policy


After information entered, review cart and proceed to paypal; have a return and cancel page.
Customer can login and review payment and registration details anytime.

For the player:

Will require the following:

first and last names
gender
date of birth
grade
school
height, weight
select sport registering for (one or multiple)



For Coaches:

Coaches login with ID/password (all coaches are customers)
Enter scores for the sport they coached (could be multiple sports)- will need to set up automatic email reminder if scores not posted after specfic time)
Submit mid-season player evaluation for the sport they coached (could be multiple sports). Will need to set up automatic email reminder if scores not posted after specfic time

For Administrators:

View customer, player, and coach details. Should be able to download data as csv file.
Will need to be able to add, delete, or edit data from any of the above tables since paper registration applications is another means of registration.

NOTE: There is no need for web design work. Code will be integrated to existing website. Current server does not allow fsockopen and will need to use curl handlers.
