Description
Purpose
The purpose of this development is to create a reusable set of software tools for developing and deploying question/answer intensive surveys using PHP / MySQL where:
a. Some branches of question are asked or not asked based on answers in other branches.
b. Additional sets of questions related or not related to each other can be added at a later date:
1. Without damaging the original data
2. Possibly creating new relationships which can require a previously un-asked question to be asked
c. Answers can be data or attachments.
d. Three functional pieces are required: Script Writer, Script Player, Script Reporter
Code and template files will be compliant with PHP Smarty Template/Framework and may include additional common support file types such Java. Where possible, open source standards and script use is encouraged if commercial license for those parts is less than $200/yr. This additional cost must be identified in your bid.
This development is a proof of concept and will be used to develop performance metrics prior to developing a full user interface and application.
User Interface shall be a browser based PHP 5.2.10 application based on Smarty 2.6.26 template/presentation framework using a MySQL 5.4 database. The Q/A Script and Responses will be stored in a database table as normal or SHA-256 encrypted data. Encryption shall be toggled on or off by a variable called by a user interface. User interfaces shall utilize strict CSS.
Multi-Language Support: Each phrase the user sees will be pulled from a language table.
Script Writer
The Script Writer will provide a simple UI to allow the user to build the Q/A Script.
Layout
The developing Q/A Script will be displayed in a File/Folder Structure on the left column “Question Tree”.
The current question the user is working on will display in the right column.
Drag&Drop question “Wizards” are displayed across the top.
(Concept drawings provided to awarded coder).
Operation
The user will drag a Wizard on top of a question in the Question Tree. A new question will form under the selected question. The new question will show the available configurations for the this Wizard type.
Alternately the user may click to select a question in the Question Tree and click to select a Wizard. First layer of questions are question categories. Categories and branches can be collapsed/expanded.
Data to collect for each question
Question Name. Instructions to user. Reference number/notes (text field).
Type of Question (a. Category, b. Single/Multiple Checkbox, c. Fill in the Blank, or
d. Supply Image/other file type)
Does the answer require another question(new branch)? If yes, prompt for new question.
Is it the end of this branch? If yes, prompt for next question to ask or end.
(Operator can choose to orphan a branch and come back to it)
Logic/Calculations
Alert operator of orphan branches which have no assigned end or next question. (Highlight question red)
Automatically Generate a Question Number for logical branch/question identification
such as software numbering (1.2.10). Keep a running count of Categories and Questions.
Script Player
The Script Player will provide a UI to prompt the user through the script.
Operation
Ask User to select from available Q/A scripts. Prompt the user through selected script.
Multiple concurrent questions which just collect information can be displayed on the same page.
Use pagination when more than 5 concurrent questions are to be displayed simultaneously.
Script Reporter
The Script Reporter will preview the questions and answers in a simple report for printing. The report will only show the questions which were answered. Provide sample data to test all possible combinations. Provide one sample script using encryption with 30 categories, 100 questions each category(duplication ok) and 10 picture questions per category.
