Hello,

I need a simple script developed that will allow me to capture lead data and then export it.

Please only bid if you are ready to begin work right away.

The script would be based on a simple form that I should be able to put on any website. The form would have the following data:

First Name
Last Name
State
Country
Email
Phone Number

What I need this script to do is to store the lead information into a database.

I would then need some simple administrative functions to then get this data to me in a form that I can use. I would like a button that would download the lead data in CSV format.

NOTE: I plan to download lead data once a week, so each time it should only download new leads since last download.

I would also like a way to download all leads should I have a need for that in the future. Some general lead stats would also be nice:

Total Leads
New Leads Since Last Download
Last Download Date

Nothing fancy, just something functional.

Also, keep in mind that I will need to have the form on many different domains, but I want one central admin area to download the leads and see the stats.

I am sure a good coder can knock this out easy. :)
